Contracts for Soloist (with Travel)

Hi, All!
 
We're flying in a soloist for the first time, and if anyone has a copy of a generic contract they've used for such an ocassion, I'd love to see it.  We just want to make sure we didn't miss anything.  Travel is a new thing for us.
 
We'll be flying the soloist in, paying for hotel and car, and a set rate for the listed rehearsals/performances/appearances, but not gas and board, etc if that helps.

I have a rider but I'll have to dig around and find it.  You could probably do a search in the internet and come up with some examples.
When I toured here are some of the specifics that were important to me particuarly and this is somewhat up to the individual performers.
Also, usually the performer has the rider/contract, not the venue.
 
Paid fees plus expenses (usually not food, sometimes the venue would provide me with dinner)
A room to dress in, with a mirror and a bathroom close by.
Bottled water in the dressing room.
Specific times for sound check with the person who WILL be running sound. Not someone who is a fill in for just rehearsal  (A BIGGIE)
Sound check will happen up until 1 1/2 hours before the performance.  After that I will not appear on stage until the performance time.  (something that I needed personally, down time before going on)
A hotel with indoor room entrances, security and a lighted parking lot.  (Safety first!)
Album sale proceeds agreement.  Most of the time I would get 100% of the sales. Sometimes I would give them a percentage, depending on the venue and the size of the audience.
Two people to man my album table. In exchange for their labor they would get a free concert ticket and a free album. 
A map and directions to the venue, and the hotel (with specific instructions for checking in if they were booking the hotel), specific times I was expected to arrive and be at the venue, check in times and an emergency contact number that I could call in case something went wrong during travel time.
Sickness, Death, Accidents or major travel emergencies (like a weather crisis) I was not responsible for. If the show had to be rescheduled I did not have to pay the cost of that, the venue did.
 
I think that is it.
I toured for a lot of years with those specific points in my rider and had very few problems.
