Plan books, grade books

Like many schools, mine insist that teachers use a plan book (either paper one or online) and a grade book (paper one to supplement the online grade postings).  In elementary, I used a regular plan book and made it work for me, but with 4 periods of choir and an exploratory music class this year, that plan book meant for elementary is not going to work. 
 
Has anyone designed a format for a 3-ring binder that works for them?
 
Also, can anyone recommend a decent paper grade book? I'm planning to grade on participation points, written tests, sight-singing and part-singing tests, and performances.  Or, if you designed one for a 3-ring binder that works, would you please share?

Update: I found a plan book that will work! I had to find one online as there wasn't anything at our local teachers' store, and designing one was my only other choice. But I'm still thinking about a grade book -- I may have to design one of those, too.
