Aloha,
I'm wondering if anyone has any audition forms they'd be willing to share with me. I'm updating mine and thought it might be helpful to see what others are doing. This is for an entry-level, general high school mixed chorus. No need to recreate the wheel, right? If you've got a form you're willing to share I'd love to see it. Thanks!

Just saw this post. I don't know how helpful this will be, since we have a small ensemble of adult professionals vs a school environment, but here's what we put on our forms:
Name & contact info
Vocal Range
(Alto, Mezzo, Sop)
Vocal Range
(Lowest comfortable note)

I have a version of the sheet for students who I have had in previous years and a form with letters of recommendation for students who have never taken choir previously. Enjoy!

The benefit is all information collected is merged into an excel-type spreadsheet. You can add information as you audition: sight-singing scores, notes on timbre, range, etc. It's all stored electronically and automatically, so no losing forms or having to type them in at the end of an already long audition process.
